Requirements for those seeking an Underground Utility license for the first time.

1. Documentation of completion of PSI exam.
2. $1,000,000 Certificate of Liability Insurance indicating CRLB as the certificate holder.
3. $20,000 bond, indicating CRLB as the certificate holder.
4. Certificate of Worker’s Compensation Insurance indicating CRLB as the certificate holder, must be submitted by applicants with employees.
5. Business ID# and date of incorporation for Corporations and Limited Liability Companies. Search a Rhode Island Secretary of State Business ID #.
6. $200 initial license fee for 2 years.

The fee can be paid online. Alternatively, checks made payable to RICRLB (please include the OpenGov Application # on the check) can be mailed to:
